Candidate Statement of Qualifications Form

The following items must be submitted to the Marin County Elections Department:

1. Any additional materials such as endorsements or certificates
2. Payment for the Candidate Statement, recorded on check as NTE as instructed in the Candidate Guide, made payable to the Marin County Elections Department

If you are running for office in a multi-county district, the above items must be submitted to the Marin County Elections Department within 72 hours of filing the statement electronically.

1. If returning by regular mail please use PO Box E, San Rafael, 94913.
2. If returning by express or next day service (non-USPS) please use 3501 Civic Center Drive, Room 121, San Rafael, 94903.
3. If you wish to authorize a third party to deliver in person you must include with your packet a Letter of Authorization with your original signature.

I acknowledge it’s my responsibility to prepare and deliver my statement to be printed in the Marin County Voter Information Guide, as well as payment for the statement, and all related materials by the close of the nomination period. I understand that if any of the aforementioned is not filed in the time prescribed, my statement will not be printed.

I will abide by the prescribed rules and procedures of the Marin County Elections Department that will also include method of filing, statement formatting, and content requirements.
